Guided Care is evidence-based, comprehensive, coordinated, proactive, longitudinal, patient-centered health care for patients with multiple chronic conditions.  This six-week online course gives nurses the knowledge and skills needed to assess patient needs, monitor chronic conditions, and educate and empower patients to improve health and quality of life.  Participants will complete about 20 hours of core content and learning activities including five live webinars, and have access to an additional 20 hours of supplemental learning activities. Click here to see a demonstration of how to navigate this online offering and review the technical requirements.

Registered nurses (RN) are eligible to take the Guided Care Nursing course and are eligible to take the online examination leading to the American Nurses Credentialing Center's new Certificate in Guided Care Nursing upon completion of the course.  Please note that the ANCC Certificate is not a certification, but rather a one-time recognition of professional achievement. Tuition includes all course materials and activities, the online exam fee, and the ANCC Certificate.   The Guided Care certificate examination will be taken at a Prometric TM testing center.  IJHN will provide detailed instructions on the required ANCC process as the Guided Care course nears completion. Upon applying for the ANCC examination, ANCC requires the CE certificate of course completion for evidence of completion of the course as an eligibility requirement. Additional information about the location of test centers, including addresses and telephone numbers of each center, is available at www.prometric.com/ancc.

NGNA and CMSA current members are eligible for discounted tuition of $1500.  Please register online and use this form to submit your payment.  Contact us for additional course information.

All payments must be in a week prior to the start of the course.
